I'm fairly sure that is Alexis Livingston. If you Google "Alexis Livingston" shemale you should pick up 3 different versions of the same video scene, though oddly, none has the same couch as the one on the box cover. And despite the German sound-over, I'm pretty sure that's San Francisco around 1980.
