So the wish comes first. If you have the wish, then of course you also wish your features would help you along - not everyone is blessed with a femme face and small bodies. In my case, I'm a crossdresser, and I have a masculine face, broad sholders, six feet tall, etc. I'm not passing at midnight at a braille-readers convension. I really wish I was more femme looking, and younger, to boot. But I have no interest in being transgender, I just like to put on female appearance. From all I've learned, being tg isn't a choice, so looks really don't matter. The tg people I've met in 'real life' basically looked like guys with long hair, but they didn't care about the looks. It's when you talk to crossdressers that you hear about passing and looking good. I think it's more important to them because they don't already "know" they're female, like true tg's do. My .02. And if you can get me some "female pills" to make me a hot tranny MILF for the weekend, let me know. :D
