Well I'm no expert but if you are serious i would think the best place to start would be to seek medical advice and help, you will probably be referred to a psychiatrist first to make sure this is what you really want, you are still very young and things might change for you.
Also talking to tgirls who have been through it all might be a good idea.

Can you find other transsexuals or transvestites in your area? That may be a better way than through the social system who doesn't normally know how to deal with trans-tendencies.

Also surf the internet and find informative web-sites - there are many. And befriend some transsexual women here on the internet as well.
