Well I'm no expert but if you are serious i would think the best place to start would be to seek medical advice and help, you will probably be referred to a psychiatrist first to make sure this is what you really want, you are still very young and things might change for you.
Also talking to tgirls who have been through it all might be a good idea. Just my opinion.
