Personal choice really.
In porn, a lot of fans prefer large breasts over small, even with the aesthetic problems so to that extent there's a bit of pressure. Also fan's desires to see cumshots means a lot of TS performers either cycle hormone doses or don't use them all together which affects breast growth and makes implants more desirable.
