Everybody is self-conscious at times. I don't think many TS girls can truly say that they see "nothing but females" looking back at them in the mirror.

Most people whom you would consider shemales think of themselves as transsexual women. As transsexuals most of them would do anything to have been born with a female body. Often this means that trans-women become fixated on their genitalia and see it as nothing but a birth defect. Many of them actually do porn to get the money to get SRS.

The majority of transsexuals never actually follow through on getting the surgery, but this is mostly for financial reasons or a general aversion to surgery. It's not usually because they don't want a vagina. You're not going to talk to any TS woman who hasn't at some point extensively fantasized about getting rid of her penis.

Personally I don't hate my penis, and I don't see any reason to get a man-made vagina without ovaries, without the ability to carry a child, and with questionable sexual function. TS women who get the surgery have to dilate it regularly to have any hope of actually fitting anything in there, and it doesn't typically self-lubricate adequately. For many, though, it's very disturbing to be a woman with a manly bulge in your panties, and that makes it worth it.

Also, the medical community in general does not see long-term hormone therapy as a good idea without getting your testicles removed. Not that there's any evidence that this is dangerous if properly monitored, but it's just not recognized that any TS woman would want to keep her testicles.
