It’s possible that it’s “photoshopped” but she looks very small in stature so it may be that she is just very “well endowed.” That's what I would guess.

Silicone injections

Silicone implants (not just for breasts anymore)

Lots and lots of “penis pumping”

Synthol. Some bodybuilders inject this into their muscles (especially arms) to gain ridiculous “size.” I have heard of synthol being injected into the penis as well. Very dangerous IMHO.
