A question about areolas (areolai?)

I have noticed this on certain t-girls, and I wonder if you folks have any answers. On some of the tgirls with larger breasts, it almost appears that you can see their original areolas within larger 'new' areolas. I was wondering - is this the work of surgery or tattooing, or is it that their areolas grow as a result of hormones?
