The question posed in this thread is fraught with difficulties. As others have pointed out, you'd have to be very clear on your definition of "real ladyboy" in order to then identify a "fake". As so often, categorization is imprecise and not that helpful. One person's definition does not necessarily correspond with another's. Does "ladyboy" here refer to the western term for Thai kathoey, or is it used as a catch-all for MtoF transgender people? Do you mean transvestite as "fake" v transsexual as "real"? Pre-op v post-op? etc, etc, etc. As was pointed out by someone in another thread, not so long ago in Thailand before readily available hormones and surgical enhancement "ladyboy" would have included effeminate gay males crossdressing as well as those identifying as women, and any amount of variety in between, so the simple 'breasts v no breasts' doesn't really help.
I'd agree with what hankhavelock says about "fake".

Though this does raise the question where one would stand with the hypothetical situation where a post-op MtoF transsexual cam model might use a fake penis because she thinks it would drum up more business
Probably best not to fret about it too much. Go for what you like, what attracts and moves you; and if you feel you are being deceived then move on.
